PRIMARY CARE
Primary Care is the anchor program for HCCH. We offer a wide range of medical services to those without other source of Primary Care. Since becoming part of the Orange County Primary Care Access Network (PCAN) we have expanded our coverage to include those in the impoverished range of 150% of poverty level in the downtown Orlando zip codes. We have not, however, forgotten our focus on the homeless patients who come from many sources; including the major city shelters, such as Coalition for the Homeless, The Salvation Army, and the Orlando Union Rescue Mission. We also provide medical services for patients in transitional housing, safe houses, rehabilitation programs, and Pathways to Care (for which we are the main primary care provider). The remainder of our patients are living in the streets, underdeveloped woods, or in cars/vehicles or doubling up with family and friends.

Our care for this fragile population continues to be holistic in approach. We try to address both body and mind. It begins with our outreach and case management then continues with care, information and education presented by our primary providers at every health visit. During any routine visit we also try to address general health issues, social health, mental health, and of course nutrition.
